Monitor Requirements/Questionaires

I was curious to know if anyone has a good process to gather monitoring requirements from different departments, or if you are a managed service provider, from your customers?  Do you use a form that is given out to the department/customer, or do most departments/customers already have a list of metrics they require?  What is your information gathering process?  If there is any advice, or examples you use and would like to share, I would greatly appreciate it.  Thanks.
