Sorry for the delay in response. At present creator information available only in the Audit Logs. We will track this as a feature request and look at the possibility of including this information in monitor dashboard in future. Meanwhile, you can manually add such monitor related information as a note in Inventory tab of monitor dashboard.

A good way to work around this is to have the creator add a note as to who requested and a link to the requesting ticket if available.
